Body planes

In healthcare, we use medical terminology to communicate a precise message to others. Earlier in the chapter, medical terminology word parts were discussed. Medical terminology also includes directional terms, planes, body cavities, and abdominopelvic quadrants and regions. All of these terms are used in healthcare to give precise descriptions and are based on a standard frame of reference, the anatomic position. The anatomic position means the body stands erect with the face forward, arms at the sides, palms forward, and toes pointed forward.

The anatomic position serves as the standard reference point for describing body locations, movements, and structures.

The body can be divided into planes using the anatomic position as another frame of reference. Planes are imaginary cuts or sections through the body. The use of plane terminology is common when discussing diagnostic imaging (e.g., computed tomography [CT] scan) of the body. Diagnostic imaging will be discussed in more detail later in the chapter.

Definitions
Coronal plane (frontal plane)
Divides the body into front and back portions. The front of the body is referred to as the anterior or ventral. The back of the body is referred to as the posterior or dorsal.
Midsagittal plane (median plane)
Separates the body into equal right and left halves. The medial line starts at the top of the head (cranium) and continues down the body to between the legs to the ground. This is also referred to as the midline of the body. Closer to the midline is referred to as medial, and farther away from the midline is called lateral.
Transverse plane (horizontal plane)
Divides the body horizontally into upper and lower sections. The area above the plane is called superior (pertaining to upward) or cephalad (toward the head). The area below the plane is called inferior (pertaining to downward) or caudad (toward the tail).

Directional and positional terms

Besides the directional terms discussed in the prior section (e.g., anterior, posterior, superior), additional terms can be used to indicate a position on the patient. Here are some additional positional and directional terms:

Unilateral
Pertains to one side.
Bilateral
Pertains to two sides.
Dextrad
Means toward the right side.
Sinistrad
Means toward the left side.
Afferent
Pertains to carrying toward a structure.
Efferent
Pertains to carrying away from a structure.
Ipsilateral
Pertains to the same side.
Contralateral
Pertains to the opposite side.
Superficial
Toward the surface of the body.
Deep
Away from the surface of the body.
Proximal
Pertains to near the origin or toward the trunk of the body.
Distal
Pertains to far from the origin or farthest from the trunk of the body.
Supine
Lying face up.
Prone
Lying face down.
Lateral position
Lying on one’s side.

Real world examples

  • The right leg is ipsilateral to the right arm but contralateral to the left arm.
  • The skin is superficial, whereas bones are deep in the body.
  • The toes are distal to the knee, while the hip is proximal to the knee.
  • The afferent arteriole brings blood to the glomerulus; the efferent arteriole carries blood away from the glomerulus.

Body cavities

The body contains cavities or hollow areas that are filled with organs. The body is separated into the dorsal (posterior) and ventral (anterior) body cavities.

The dorsal body cavity protects the nervous system organs and contains the following:

Contains the brain; surrounded and protected by the cranium (skull).
Spinal cavity
Contains the spinal cord; surrounded and protected by the vertebrae (bones of the spine).

The ventral body cavity consists of two cavities that are divided by the diaphragm:

Thoracic cavity
Contains the heart, lungs, esophagus, and trachea (windpipe). Protected by the ribs, the sternum (breastbone), and the vertebrae (backbones).
Abdominopelvic cavity
Can be divided as follows:
Abdominal cavity
Contains the abdominal organs (e.g., stomach, liver, gallbladder, intestines).
Pelvic cavity
Contains the urinary bladder and the reproductive organs.

Abdominopelvic quadrants and regions

The abdominopelvic cavity is extensive. Either the four quadrants or the nine regions can be used to help describe a location in the abdominopelvic area. Descriptions that focus on the quadrants are simpler to understand and may be used with patients. The regions are more specific and are typically used by healthcare providers.

With the abdominopelvic quadrants, an imaginary line is drawn down the midline of the body. A horizontal line is drawn across the abdominopelvic cavity, intersecting at the navel. These quadrants are referred to as either right or left and upper or lower. Typically, abbreviations are used when documenting information about the patient.

Definitions
Right upper quadrant (RUQ)
Includes the right lobe of the liver, gallbladder, right kidney, small intestine (duodenum), large intestine (ascending and transverse colon), and head of the pancreas.
Left upper quadrant (LUQ)
Includes the stomach, spleen, left lobe of the liver, pancreas, left kidney, and large intestine (transverse and descending colon).
Right lower quadrant (RLQ)
Includes the appendix, cecum, right ovary, right ureter, right spermatic cord, and large intestine (ascending colon).
Left lower quadrant (LLQ)
Includes the small intestine, large intestine (descending and sigmoid colon), left ovary, left ureter, and left spermatic cord.

The nine abdominopelvic regions lie over the abdominopelvic cavity. They provide a more specific location than the quadrants.

Right hypochondriac
The upper right region, under the ribs. Includes the right lobe of the liver, the gallbladder, and part of the right kidney.
Epigastric
The upper middle region, above the stomach. Includes the stomach, part of the liver, the pancreas, the duodenum, and the adrenal glands.
Left hypochondriac
The upper left region, under the ribs. Includes the spleen, part of the stomach, the tail of the pancreas, and part of the left kidney.
Right lumbar
The middle right region. Includes the ascending colon, the right kidney, and part of the small intestine.
Umbilical
The middle region, around the navel. Includes the transverse colon, the duodenum, and part of the small intestine.
Left lumbar
The middle left region. Includes the descending colon, the left kidney, and part of the small intestine.
Right iliac (inguinal)
The lower right region. Includes the cecum, the appendix, and the beginning of the ascending colon.
Hypogastric (pubic)
The lower middle region. Includes the urinary bladder, the sigmoid colon, part of the small intestine, and the uterus in a female patient.
Left iliac (inguinal)
The lower left region. Includes the sigmoid colon and part of the descending colon.

The regions sit in three rows of three, like a tic-tac-toe grid laid over the abdomen: the two hypochondriac regions with the epigastric between them across the top, the two lumbar regions with the umbilical between them across the middle, and the two iliac regions with the hypogastric between them across the bottom. As with the quadrants, right and left always mean the patient’s right and left.

Anatomical planes, directions, and body cavities

Body planes

In healthcare, we use medical terminology to communicate a precise message to others. Earlier in the chapter, medical terminology word parts were discussed. Medical terminology also includes directional terms, planes, body cavities, and abdominopelvic quadrants and regions. All of these terms are used in healthcare to give precise descriptions and are based on a standard frame of reference, the anatomic position.
