Introduction: How to Study Conditions and Symptoms for the MBLEx
This chapter introduces common conditions and symptoms that may appear on the MBLEx and that massage therapists may hear about in practice. The goal is not to train massage students to diagnose. Diagnosis belongs to appropriately licensed medical professionals. The goal is to help beginning massage therapists recognize the basic language of pathology, understand how different conditions are organized by body system, and think safely about when bodywork may need to be modified, delayed, or referred out.
A useful first step is to notice that condition names often contain clues. A word ending in -itis often suggests inflammation, as in bronchitis or bursitis. A word ending in -osis often suggests a condition or process, such as scoliosis or atherosclerosis. Sclerosis refers to hardening. Osteo- points toward bone. Neuro- points toward nerves. Myo- points toward muscle. These word parts do not tell the whole story, but they help students approach unfamiliar terms with less fear.
Another important study habit is to separate the condition itself from the symptom a client reports. For example, numbness and tingling in the hand could be related to carpal tunnel syndrome, thoracic outlet syndrome, cervical radiculopathy, peripheral neuropathy, or another medical problem. A massage therapist does not need to determine the exact diagnosis before every session, but must know that altered sensation can make deep pressure, heat, cold, or aggressive techniques unsafe in the affected area.
Throughout this chapter, pay attention to three kinds of testable information: the name of the condition, the structure or body system involved, and the basic pattern of signs or symptoms. When a condition is infectious, progressive, life-threatening, associated with altered sensation, or associated with fragile tissues, that safety information is especially important for massage practice.
Bone and skeletal structure
The skeletal system gives the body support, protects vital organs, stores minerals, and provides attachment sites for muscles. Bone may feel solid and unchanging, but living bone is active tissue. It is constantly being built, broken down, repaired, and remodeled. Many skeletal conditions involve a problem with this balance between building bone and removing bone. Others involve infection, growth-related irritation, spinal alignment, or overuse injuries.
Bone density and bone strength
Osteopenia means low bone density that is not severe enough to be called osteoporosis. It can be thought of as a warning zone: bone mineral density is reduced, but the loss has not crossed the diagnostic threshold for osteoporosis. For MBLEx purposes, the important distinction is that osteopenia is less severe than osteoporosis but still tells the massage therapist to be thoughtful about pressure, positioning, and fall risk.
Osteoporosis literally means “porous bones.” In osteoporosis, minerals such as calcium are removed from bone faster than they are replaced. The result is thinner, weaker, more brittle bone that is more vulnerable to fracture. A client with osteoporosis may not look fragile from the outside, so intake questions matter. For massage practice, the basic safety concern is that strong compressive force, aggressive stretching, or sudden twisting may be inappropriate for fragile bones, especially in older adults or clients with known low bone density.
Osteomalacia means bone softening. It differs from osteoporosis because the problem is not simply that previously healthy bone is being dismantled too quickly. Instead, the bone-building process itself is impaired, leaving bone insufficiently hardened. When this condition appears in children, it is traditionally called rickets. The MBLEx-relevant distinction is: osteoporosis is often described as porous, brittle bone from mineral loss; osteomalacia is softening related to poor bone mineralization or rebuilding.
Bone infection and bone death
Osteomyelitis is infection of bone. It may be acute or chronic. Because bone infection can be serious, painful, and medically complex, it is not a routine “sore area” for massage to address directly. A student should remember the word part: osteo- means bone, and -myelitis in this context refers to infection/inflammation within bone or marrow-related tissue.
Osteonecrosis, also called avascular necrosis or AVN, is death of bone tissue caused by loss of blood supply. Bone, like other living tissue, needs circulation. When blood flow is insufficient, the affected bone tissue can deteriorate. If untreated, the condition may lead to pain, loss of function, and even collapse of the involved bone surface near a joint. The testable core is that osteonecrosis/AVN is bone death due to inadequate blood supply.
Paget’s disease of bone
Paget’s disease of bone, also called osteitis deformans, is a disorder of abnormal bone remodeling. Bone is resorbed too rapidly because osteoclast activity is increased. The body then replaces the affected areas with disorganized tissue rather than strong, normally organized bone. Over time, affected bones may become enlarged, distorted, and weakened. Paget’s disease is most common in people over age 50. The key test points are increased osteoclast activity, abnormal remodeling, fibrous or disorganized replacement tissue, and weakened or deformed bone.
Spinal curves and spinal stress injuries
Scoliosis is a postural or structural deviation in which the spine curves laterally, meaning side-to-side, often in a C-shaped or S-shaped pattern. Some scoliosis also includes rotation of the vertebrae. When scoliosis includes spinal rotation, it may be called rotoscoliosis.
Spondylolysis is a crack or stress fracture in a vertebra, usually in the lumbar spine and often at L5. The word ending -lysis can help students remember a breaking, loosening, or separation. This condition can impair the weight-bearing capacity of the spine and may contribute to back pain. It is different from spondylosis, which is degenerative arthritis of the spine, and different from spondylolisthesis, in which a vertebral body slips forward.
Growth-related and overuse conditions of the lower leg
Osgood-Schlatter disease, also called tibial tuberosity apophysitis, is an irritation and inflammation at the tibial tuberosity, where the quadriceps attach to the tibia. It usually occurs in active children or adolescents during rapid growth of the leg bones. The testable image is a young active person with pain or irritation at the quadriceps attachment on the tibia, near the front of the knee.
Medial tibial stress syndrome, or MTSS, is the most common cause of the anterior tibial pain often called shin splints. It is an overuse condition. One proposed mechanism is irritation or damage to the periosteum or cortical bone related to overloading by muscles such as soleus and tibialis posterior. For exam purposes, connect MTSS with shin splints and repetitive loading of the tibial region.
A tibial stress fracture is a hairline fracture or bone bruise of the tibia. It may also produce shin-splint-like pain, but it is more specifically a stress injury to bone. It is an overuse injury and may be visible on MRI even when an X-ray does not show it clearly. A practical distinction is that MTSS is often discussed as a soft tissue/periosteal overuse syndrome, while a tibial stress fracture is a bone injury.
Circulatory system
The circulatory system includes the heart, blood vessels, blood, and the movement of fluids through tissues. Conditions in this section often involve blood pressure, blood flow, clotting, vessel health, or fluid accumulation. Massage students should be especially cautious with circulatory conditions because pressure, heat, positioning, and vigorous movement can affect circulation.
Bleeding and clotting
Hemophilia refers to a group of genetic disorders in which important plasma proteins needed for clot formation are missing or deficient. Because clotting is impaired, bleeding may be prolonged or excessive after injury. The testable idea is not simply “bleeding disorder,” but specifically a genetic clotting disorder involving missing clotting proteins.
A heart attack, also called a myocardial infarction or MI, occurs when blood flow to part of the heart muscle is blocked. The blockage is often related to a clot or plaque buildup in the coronary arteries. Without oxygen-rich blood, the affected heart muscle becomes damaged or dies. This is a medical emergency. In exam language, myocardial means heart muscle, and infarction means tissue death due to insufficient blood supply.
Vessel hardening and plaque
Arteriosclerosis is a broad term meaning hardening of the arteries from any cause. Arteries normally need to be strong but also somewhat elastic. When they become hardened, circulation can be compromised.
Atherosclerosis is a specific type of arteriosclerosis involving deposits of cholesterol, calcium, and other substances in the walls of large and medium-sized blood vessels. These deposits can narrow the vessel and can also increase the risk of clot formation and clot release. A helpful distinction is that all atherosclerosis is a form of arteriosclerosis, but arteriosclerosis can refer to arterial hardening from causes other than plaque deposits.
Blood pressure and fainting
Hypertension means high blood pressure. Students should not confuse hypertension with hypertonicity. Hypertension refers to pressure inside blood vessels; hypertonicity refers to increased tension in muscle tissue. Hypertension may be chronic and medically managed, or it may be part of an acute medical problem. Massage therapists should understand the term and be attentive to client reports of uncontrolled blood pressure.
Syncope means fainting. It is a temporary loss of consciousness and muscle tone caused by a sudden drop in blood flow to the brain. It often resolves spontaneously, but it may be preceded by dizziness, nausea, sweating, or light-headedness. Syncope can be benign, but it can also signal a serious underlying issue. A client who feels faint during a session should be treated as needing immediate supportive attention: stop the session, keep them safe from falling, and follow appropriate emergency procedures if symptoms do not resolve or if red flags are present.
Edema and lymphatic swelling
Edema is the general term for swelling, regardless of cause. The swelling may come from venous problems, lymphatic problems, injury, inflammation, medication effects, pregnancy, heart or kidney disease, or other causes. Because swollen tissues may already be under fluid stress, many types of edema require very gentle work or avoidance of direct pressure unless the therapist has the appropriate training and the condition is appropriate for massage.
Pitting edema is a sign in which a depression remains in the tissue after pressing with a thumb or finger. The remaining “pit” suggests disrupted blood or lymph flow. This is testable because it is an observable sign, not a diagnosis by itself.
Lymphedema is swelling caused by insufficient lymphatic system function. It may develop after disease, injury, surgery, or medical treatment. Because lymphatic capillaries are delicate and the system may already be overloaded, lymphedema is not treated with ordinary deep massage. When massage is appropriate, it requires specialized training and gentle techniques designed for lymphatic care.
Digestive system
Digestive conditions often involve the gastrointestinal tract, nutrient absorption, inflammation, or immune reactions to food substances. The massage therapist is not treating the digestive disease directly, but should understand basic terms, recognize when abdominal work may be inappropriate, and notice whether symptoms suggest a need for medical care.
Diverticular disease
Diverticular disease refers to a spectrum of conditions involving small pouches called diverticula that form in the lining of the colon, or large intestine. When diverticula are present without symptoms, the condition is called diverticulosis. When the pouches become inflamed or symptomatic, the condition is called diverticulitis. The most important MBLEx distinction is the difference between the suffixes: -osis may suggest the presence of the condition, while -itis indicates inflammation.
For massage students, diverticulitis is the more acute concern. Abdominal pain, fever, significant tenderness, or known active inflammation should not be treated as ordinary muscle tension. Abdominal work should be avoided when there is acute inflammation, unexplained pain, or medical instability.
Celiac disease
Celiac disease is a chronic autoimmune disorder in which ingestion of gluten, a protein found in wheat, barley, and rye, triggers an immune response that damages the lining of the small intestine. Because the small intestine is the major site of nutrient absorption, this damage can impair the body’s ability to absorb nutrients properly. Symptoms can include gastrointestinal complaints, fatigue, nutritional deficiency, and other systemic effects.
The testable core is that celiac disease is autoimmune, gluten-triggered, and damaging to the small intestinal lining. It is not simply a food preference and not the same thing as a non-celiac gluten sensitivity.
Injuries
Injuries are among the most important pathology topics for massage therapists because clients frequently seek massage for pain, stiffness, or recovery after physical stress. The MBLEx often tests whether a student can distinguish a ligament injury from a muscle or tendon injury, and whether the student understands the basic grades of tissue damage.
Skin injury
A laceration is a rip, tear, or cut in the skin. Because lacerations involve an opening in the protective barrier of the skin, direct massage over the area is inappropriate until the tissue has healed sufficiently. Open wounds also raise concerns about infection control.
Bruises, contusions, and hematomas
A contusion is a bruise. It occurs when capillaries rupture and blood diffuses into extracellular spaces where whole blood is not normally found. Bruises often change color as the body breaks down and clears the leaked blood.
A hematoma is more extensive bleeding and pooling of blood in a confined area. The pooling is what distinguishes a hematoma from a simple contusion. The word literally suggests a blood mass or blood tumor, although it is not a cancer. A hematoma is internal bleeding; it is not the correct term for an open skin injury where blood escapes outside the body.
Sprains and strains
A sprain is a tear or injury to a ligament. Ligaments connect bone to bone and help stabilize joints. When a ligament is stretched beyond its capacity, fibers may be damaged or torn.
A strain is a tear or injury to muscle tissue or to the tendon associated with the muscle. Tendons attach muscle to bone, so the forces that strain a muscle may also irritate or injure its tendon. Strains can be involved in injury-reinjury cycles because scar tissue may be weaker and less organized than the original tissue, especially if the area is repeatedly overloaded before it has recovered.
Sprains and strains are commonly described by grade. A grade 1 or first-degree sprain or strain involves stretching with minimal damage to the tissue fibers. A grade 2 or second-degree sprain or strain involves a partial tear. A grade 3 or third-degree sprain or strain involves a complete tear. For the exam, remember: sprains affect ligaments; strains affect muscles or tendons; grade 1 is mild stretching/minimal fiber damage; grade 2 is partial tear; grade 3 is complete tear.
Joints
Joints are places where bones meet. Many joint conditions involve cartilage, synovial membranes, joint capsules, bursae, tendons, discs, or the alignment of bones. Because massage therapy often works around joints, students must understand the difference between joint inflammation, degenerative change, displacement, and nerve irritation related to joint structures.
Arthritis and inflammatory joint disease
Osteoarthritis is a condition in which synovial joints lose healthy cartilage because of wear and tear related to activity, age, or mechanical stress. As cartilage becomes less healthy, joint surfaces may move less smoothly, and pain or stiffness may develop. Osteoarthritis is often described as degenerative joint disease.
Rheumatoid arthritis, or RA, is an inflammatory autoimmune condition in which the immune system attacks the synovial membranes of joints. Unlike osteoarthritis, which is primarily degenerative and mechanical, RA is systemic and immune-mediated. As RA progresses, it may affect other tissues and organs, including blood vessels, serous membranes, skin, eyes, lungs, liver, and heart. The exam distinction is that RA attacks synovial membranes and can have systemic consequences.
Gout is joint inflammation caused by a chemical imbalance in which uric acid forms needle-like crystals in and around synovial membranes. These crystals irritate surrounding tissues and can cause intense pain and inflammation. The classic first joint affected is the base of the great toe, where the metatarsal meets the proximal phalanx.
Ankylosing spondylitis is an inflammatory condition that leads to stiff joints in the spine. It is generally considered autoimmune. The word ankylosing suggests stiffening or fusion, while spondylitis points toward inflammation of spinal joints or vertebral structures. For the MBLEx, link it with autoimmune inflammation and stiffness of the spine.
Shoulder and upper-extremity joint conditions
Adhesive capsulitis, also called frozen shoulder, is a condition in which the connective tissues around the glenohumeral joint become inflamed, then thickened and restrictive. The result is loss of shoulder range of motion. Students should connect “capsulitis” with the joint capsule and remember that frozen shoulder involves the shoulder joint capsule rather than a simple tight muscle.
Shoulder impingement occurs when the acromion of the scapula pinches the supraspinatus muscle and/or tendon. It may also be called rotator cuff tendinitis or swimmer’s shoulder. The testable relationship is acromion plus supraspinatus, often with pain during shoulder movement.
Temporomandibular joint dysfunction, or TMJD, can cause severe jaw pain, crepitus, and joint degeneration. It may involve trigger points, internal derangement of the joint, osteoarthritis, or a combination of factors. Massage therapists may work with surrounding soft tissues when appropriate, but severe jaw pain or joint degeneration should be understood as more than ordinary muscle tightness.
Crepitus is a crackling, popping, or grinding sensation or sound in a joint. Crepitus is a sign or symptom that can have many causes. It does not name a single disease by itself.
Cysts, bunions, and toe deformity
A Baker’s cyst, also called a popliteal cyst, is a synovial cyst found in the popliteal fossa, usually on the medial side of the back of the knee. Because it is related to synovial fluid and occurs behind the knee, students should not confuse it with a skin cyst or a muscle knot.
A bunion is also called hallux valgus, meaning the big toe is deviated laterally. In this condition, the first phalanx of the great toe angles toward the lateral side of the foot. The joint capsule stretches, a bursa may grow at the irritated site, and callus may form over the protrusion. For exam purposes, hallux means great toe, and valgus here points to lateral deviation of that toe.
Displacement of joint surfaces
A subluxation is a partial dislocation of a joint. The joint surfaces still have some contact, and the joint may still be partly functional, but motion may be limited and uncomfortable.
A dislocation occurs when the bones that form a joint are forced out of their normal positions. This misalignment can damage surrounding ligaments, tendons, muscles, and nerves. Dislocations are commonly caused by trauma such as falls, blows, or sudden impacts. They typically involve pain, loss of motion, and sometimes visible deformity. A suspected dislocation is not a massage problem; it requires appropriate medical care.
Spinal joint and disc conditions
Spondylosis refers to degenerative arthritis of the spine. It involves age-related changes of the vertebrae, discs, joints, and ligaments. It happens frequently and often severely in the cervical spine. Some MBLEx practice language may use spondylosis more broadly for degenerative vertebral conditions, but the core association is degeneration in the spine.
Spondylolisthesis is a condition in which a structural problem in the lumbar spine allows one or more vertebral bodies to slip anteriorly. It can involve small or large bone fractures and may place pressure on nerve roots at the intervertebral foramina or on the spinal cord within the spinal canal. Students should distinguish this from spondylolysis: spondylolysis is a crack or stress fracture; spondylolisthesis is slipping.
A herniated disc, also called a slipped disc or ruptured disc, occurs when the soft inner nucleus pulposus of an intervertebral disc pushes through a tear or weakened area in the outer annulus fibrosus. The herniated material may irritate or compress nearby nerves, leading to pain, numbness, or weakness in the neck, back, arm, or leg depending on the level involved. The key structures are nucleus pulposus inside and annulus fibrosus outside.
Nervous system
The nervous system includes the brain, spinal cord, cranial nerves, spinal nerves, and peripheral nerves. Nervous system conditions often involve pain, altered sensation, weakness, movement changes, or problems with coordination. For massage therapists, altered sensation is especially important because the client may not accurately perceive pressure, heat, or cold.
Nerve compression and nerve pain
Sciatica refers to pain, numbness, or tingling in the glutes, hip, thigh, or leg caused by compression of the nerve fibers that merge to become the sciatic nerve. This is sometimes called “true sciatica” to distinguish it from conditions that mimic sciatic symptoms.
Piriformis syndrome causes pain or numbness in the glutes, hip, thigh, or leg because the piriformis muscle compresses the sciatic nerve. The symptoms can resemble sciatica, but the location of compression is different. In true sciatica, the compressed nerve fibers may be closer to the spine; in piriformis syndrome, the piriformis muscle is the suspected source of compression.
Thoracic outlet syndrome, or TOS, occurs when structures such as the scalenes or pectoralis minor compress nerve fibers. Symptoms may appear in the arm and hand as numbness, tingling, pain, and sometimes motor problems. The testable anatomy is that compression occurs around the thoracic outlet region and can involve the arm and hand.
Radiculopathy is the diagnosis assigned to conditions that cause radicular pain, meaning pain that travels along the distribution of a nerve root. It is different from peripheral neuropathy because radiculopathy involves spinal nerve roots rather than damage to peripheral nerves farther out in the body.
Neuropathy, or peripheral neuropathy, is a general term for damage to peripheral nerves. It can cause numbness, tingling, weakness, autonomic dysfunction, and pain. Neuralgia refers specifically to the pain caused by nerve damage or irritation. Because sensation may be reduced in neuropathy, hot or cold hydrotherapy and deep pressure are locally contraindicated in affected regions unless specifically appropriate and safely modified.
Trigeminal neuralgia is a chronic pain condition affecting the trigeminal nerve, which carries sensory information from the face to the brain. It can cause sudden, severe facial pain described as electric shock-like, stabbing, or burning. The key testable point is severe facial pain involving the trigeminal nerve.
Movement disorders and neuromuscular conditions
Dystonia involves repetitive, involuntary, and sometimes sustained contractions of skeletal muscles. The most common form affects the neck and is called spasmodic cervical torticollis. In this condition, neck muscle contraction causes the head to be held tilted and shifted laterally. For the exam, link dystonia with involuntary contractions and torticollis with the neck.
Parkinson’s disease, or PD, is a progressive neurological disorder that primarily affects movement, although non-motor functions may also be involved. Motor symptoms include tremor, rigidity, slowed movement called bradykinesia, and postural instability that can contribute to falls. Parkinson’s disease is associated with degeneration of dopamine-producing neurons in the substantia nigra, a region of the brain important for movement regulation.
Cerebral palsy, or CP, is an umbrella term for brain injuries that occur during gestation, delivery, or early infancy. Muscular symptoms may include spastic muscles that are so tight that their antagonists let go, tremor, slow involuntary twisting movements, and writhing movements of the extremities, face, or mouth. The core testable idea is early brain injury with lifelong motor effects.
Multiple sclerosis, or MS, is a chronic autoimmune disease affecting the brain, spinal cord, and nerves. The immune system attacks the myelin covering of axons, a process called demyelination. Because myelin helps nerve impulses travel efficiently, demyelination disrupts normal nerve cell activity and can create a wide range of neurological symptoms. The exam association is autoimmune demyelination.
Bell’s palsy is sudden, temporary weakness or paralysis of the muscles on one side of the face due to inflammation or compression of the facial nerve, cranial nerve VII. It commonly causes drooping on the affected side and difficulty with facial expressions such as smiling or closing the eye. It is the most common cause of facial paralysis and often resolves over weeks to months.
Stroke and meningitis
A stroke occurs when brain tissue is damaged by insufficient blood flow or by bleeding in or around the brain. An ischemic stroke is caused by inadequate blood flow, often because a vessel is blocked. A hemorrhagic stroke involves bleeding. Either type can damage brain tissue and may produce weakness, speech problems, facial drooping, altered consciousness, or other neurological signs. Stroke symptoms are a medical emergency.
Meningitis is inflammation of the meninges, the protective membranes around the brain and spinal cord. It is most often caused by infection, including viral, bacterial, fungal, or other microbial infections. Less commonly, it may arise from non-infectious causes such as autoimmune disease or medication reactions. Bacterial meningitis can be life-threatening and requires prompt medical care. The testable core is inflammation of the meninges around the brain and spinal cord.
Headache types and massage cautions
Tension headaches can be triggered by soft tissue injury, muscle tightness, trigger point referral, temporomandibular joint disorders, eyestrain, poor ergonomics, or mental/emotional stress that worsens inefficient posture and movement. Of the headache types in this deck, tension headaches are the main type for which massage may be appropriate during acute symptoms, depending on the individual presentation.
Migraine headaches are recurrent headaches that may range from moderate to severe. They are often accompanied by nausea, sensitivity to light (photophobia), sensitivity to sound (phonophobia), and sometimes visual or sensory disturbances called aura. Many adults with migraine experience throbbing pain on one side of the head, sometimes with watering of the eye or nostril on the same side. Common triggers may include hormonal shifts, some foods such as red wine, aged cheese, or chocolate, changes in sleep, excessive exercise, and stress.
Cluster headaches are rare but extremely painful headaches that occur in cyclical patterns. Pain is usually one-sided and located around or behind the eye. Attacks may last 15 minutes to three hours and can occur several times per day for weeks or months, followed by long periods without recurrence.
Chemical headaches are caused by exposure to or withdrawal from substances that affect the brain’s blood vessels, nerves, or neurotransmitter systems. These may include environmental toxins, medications, recreational drugs, or dietary substances.
Sinus headaches are associated with inflammation or congestion in the sinuses, the air-filled cavities in the skull. They are typically related to sinusitis, meaning inflammation of the sinus lining, and often cause pressure or pain in the face, forehead, or around the eyes.
Inflammatory headaches are caused by swelling in or around the brain and its structures, including blood vessels, meninges, or other tissues. They are usually secondary to another condition, such as infection, autoimmune disease, or localized inflammation.
A key safety rule from the deck is that during acute headache symptoms, massage is contraindicated for all headache types except tension headaches. In other words, acute migraine, cluster, chemical, sinus, inflammatory, and traction-type headache presentations should not be treated as ordinary neck tension during the acute episode.
Respiratory system
Respiratory conditions affect the airways, lungs, alveoli, mucus production, or the exchange of oxygen and carbon dioxide. Massage therapists should pay attention to breathing difficulty, contagious infections, fatigue, and positioning. Some clients breathe better semi-reclined than lying flat.
Airway inflammation and narrowing
Asthma is a chronic respiratory condition involving inflammation and narrowing of the airways. It can produce episodes of wheezing, shortness of breath, chest tightness, and coughing. Symptoms may be mild or severe and are often triggered by allergens, exercise, or respiratory infections. The MBLEx association is reversible airway narrowing with wheezing and breathing difficulty.
Bronchitis is inflammation of the bronchial tubes, the branching airways that carry air to and from the lungs. Bronchitis may be acute or chronic depending on the cause and duration. The testable structure is the bronchi or bronchial tubes.
Chronic obstructive pulmonary disease, or COPD, is a progressive chronic lung disease with persistent airflow obstruction that interferes with breathing. It primarily includes chronic bronchitis and emphysema. COPD is often associated with long-term exposure to irritants such as cigarette smoke, air pollution, occupational dust, or chemicals.
Alveolar and mucus-related conditions
Emphysema is a chronic lung condition involving destruction of the alveoli, the small air sacs where oxygen and carbon dioxide are exchanged. When alveoli are damaged, the lungs lose surface area for gas exchange, making breathing less efficient. Emphysema is a major component of COPD.
Pneumonia is an infection of the lungs that inflames the alveoli. The air sacs may fill with fluid or pus. Pneumonia can affect one or both lungs and can range from mild to life-threatening, especially for infants, older adults, and people with weakened immune systems. Because it is an infection and may be serious or contagious depending on the cause, active pneumonia is not a routine massage situation.
Cystic fibrosis, or CF, is a chronic genetic disorder that causes production of thick, sticky mucus. This mucus contributes to chronic inflammation and infection in the lungs. CF also affects other systems, including the digestive, integumentary, and reproductive systems. For the MBLEx, associate cystic fibrosis with thick mucus, chronic lung infection/inflammation, and genetic origin.
Integumentary system
The integumentary system includes the skin and related structures such as hair, nails, sweat glands, and oil glands. Skin conditions are especially important in massage because the therapist directly contacts the skin. Some conditions are infectious and require avoidance of the affected area or postponement of massage. Others are non-infectious but may still require modifications because the skin is fragile, inflamed, painful, or easily irritated.
General skin signs
Erythema means redness of the skin caused by increased blood flow to capillaries in the skin. It may arise from harmless temporary causes such as blushing or heat exposure, or from irritation, infection, or chronic conditions such as rosacea or lupus. Erythema is a sign, not a single diagnosis.
Cyanosis is a bluish color of the skin or mucous membranes that suggests insufficient oxygen in the blood. Cyanosis can be medically serious. The testable association is blue color plus low oxygenation.
Pruritus means itching. It is an uncomfortable sensation that creates the urge to scratch. It can be localized to one area or related to a systemic issue. Itching may be associated with skin disorders, allergic reactions, infections, or internal disease.
Noninfectious inflammatory and autoimmune skin conditions
Dermatitis is a broad, nonspecific term meaning skin inflammation. It is often used for noninfectious skin disorders. Because it is an umbrella term, students should avoid assuming a single cause.
Eczema is a red, itchy, scaling skin reaction caused by a histamine-based immune overreaction or autoimmune response. It is not simply dry skin, although dryness and scaling may be part of the appearance.
Psoriasis is a chronic disease in which skin cells reproduce much faster than normal. Instead of being replaced about every 28 to 32 days, the cells may be replaced every 3 to 4 days. Because they do not slough off normally, they accumulate into itchy, scaly plaques, often on the scalp, trunk, elbows, and knees. Psoriasis is now considered autoimmune.
Scleroderma is hardening of the skin caused by an autoimmune reaction. In some people, it can also cause serious complications involving internal organs. For the MBLEx, remember skin hardening and autoimmune origin.
Urticaria is another name for hives. Hives are a rash with red, raised, itchy bumps or welts. They often reflect an allergic or histamine-related reaction, though triggers vary.
Bacterial and viral skin conditions
Cellulitis is a general infection of the deep layers of the skin and subcutaneous tissues, often involving streptococcal bacteria. It is distinguished from more localized infections by the absence of an abscess or draining wound. It can occur as a complication of simple injuries such as a scraped knee or a contaminated blister. Cellulitis is medically important and not an area for direct massage.
Shingles is a resurgence of a previously acquired varicella zoster virus infection. The earlier infection is most commonly chickenpox. Shingles usually appears as a painful blistering outbreak along the dermatome where the virus remained dormant. Because it follows a nerve distribution and can be painful and contagious to susceptible people through contact with blister fluid, massage therapists must use caution and follow appropriate contraindication guidelines.
Plantar warts grow on the soles of the feet and may be mistaken for callus. Warts are viral, so the therapist should avoid direct contact with the lesion and follow infection-control procedures.
Fungal infections: tinea and nails
Tinea refers to fungal infections of the skin commonly called “ringworm,” even though no worm is involved. Tinea infections are generally contagious and are named by location.
Tinea capitis is a fungal infection of the hair follicles of the scalp. Tinea corporis is body ringworm, usually appearing on the trunk or extremities. It may begin as a small, round, red, scaly, itchy patch. As it spreads, lesions may heal from the center first, creating the appearance of a ring with a raised scaly edge. Tinea cruris is tinea of the groin, also called jock itch. Tinea pedis is tinea of the foot, commonly called athlete’s foot. Onychomycosis, also called tinea unguium, occurs when a fungal infection invades the skin under fingernails or toenails.
The testable pattern is simple: tinea is fungal; capitis affects scalp; corporis affects body; cruris affects groin; pedis affects feet; unguium/onychomycosis affects nails.
Pressure injury and acne
A decubitus ulcer, also called a pressure ulcer or bedsore, is a localized injury to skin and underlying tissue caused by prolonged pressure, friction, or shear. These ulcers often develop over bony prominences where tissue is compressed between bone and a bed, chair, or other surface. Reduced blood flow leads to tissue damage. Massage therapists should never treat a pressure ulcer as if it were ordinary tight tissue; open or fragile tissue requires medical care and careful avoidance.
Acne vulgaris is a common chronic skin condition in which hair follicles become clogged with sebum and dead skin cells, leading to inflammation. It may include blackheads, whiteheads, papules, pustules, nodules, and cysts. Acne may be tender, inflamed, or vulnerable to irritation, so local work should be modified accordingly.
Other conditions
The “other” category contains conditions that cross body systems or do not fit neatly into one of the earlier categories. Many of these are highly relevant for massage because they involve soft tissue pain, tendon irritation, nerve compression tunnels, endocrine function, pregnancy complications, immune function, heat illness, or connective tissue fragility.
Tendons, tendon sheaths, fascia, and bursae
Tenosynovitis is irritation where tendons slide through their synovial sheaths. When it affects a non-thumb finger, it is commonly called trigger finger. When it affects the thumb, it is called de Quervain syndrome. The testable structure is the tendon sheath, not the joint capsule.
Bursitis is inflammation of a bursa, a fluid-filled sac that helps tendons glide smoothly over bony prominences. The body contains many bursae. When a bursa is inflamed, direct pressure may be painful and inappropriate.
Plantar fasciitis is painful degeneration of the plantar aponeurosis on the bottom of the foot. It is often felt as heel pain, pain just distal to the heel, or deep foot pain. It is commonly worse when first standing after a long period of sitting or lying down. Although the name ends in -itis, inflammation may be intermittent; the long-term condition is often more accurately understood as degeneration.
Contracture can mean either tight, fibrous, atrophied muscles caused by paralysis, or a scar in the skin that has healed so tightly that it decreases range of motion at one or more joints. In both cases, the tissue has lost normal extensibility and limits movement.
Compartment syndrome is a condition in which injury or repetitive stress creates dangerous pressure inside a tight fascial compartment, most often in the lower leg. Acute compartment syndrome can threaten the health of the limb and may even cause life-threatening kidney damage if not treated quickly. Acute cases require immediate medical intervention, often surgery. This is a major red-flag condition.
Epicondylitis and tunnel syndromes
Tennis elbow, or lateral epicondylitis, is pain at the lateral epicondyle of the humerus where the wrist extensors attach. It may also present with tingling, numbness, or weakness in the hand and forearm.
Golfer’s elbow, or medial epicondylitis, is pain at the medial epicondyle of the humerus where the wrist flexors attach. Like tennis elbow, it may also include tingling, numbness, or weakness in the hand and forearm. A simple memory tool is: tennis/lateral/extensors; golfer’s/medial/flexors.
Carpal tunnel syndrome is caused by compression of the median nerve as it passes through the carpal tunnel in the wrist. The carpal tunnel is formed by bones and ligaments and contains the median nerve and flexor tendons. When the tunnel narrows or the tissues around the tendons swell, pressure on the median nerve may cause pain, numbness, and weakness in the hand.
Tarsal tunnel syndrome is caused by compression or irritation of the posterior tibial nerve as it passes through the tarsal tunnel on the medial side of the ankle near the medial malleolus. The tarsal tunnel is covered by the flexor retinaculum and contains nerves, arteries, and tendons. Symptoms may include pain, burning, or tingling along the inner ankle or sole of the foot.
Spinal stenosis is narrowing of the spinal canal or the neural foramina, the spaces where nerves exit the spine. This narrowing can compress the spinal cord or nerve roots and may cause pain, numbness, or weakness in the back, neck, legs, or arms depending on the location. Stenosis means narrowing.
Pain syndromes and post-exercise soreness
Fibromyalgia syndrome is a chronic condition involving widespread tender points in soft tissues. Its cause is not well understood. It often includes sleep difficulties and hormonal dysfunction. Massage therapy may offer relaxation that helps some clients improve sleep quality, but pressure tolerance can vary widely.
Complex regional pain syndrome, or CRPS, is a chronic pain condition that usually develops after an injury, surgery, stroke, or heart attack. Pain is persistent, severe, and disproportionate to the original injury or trauma. It often affects an extremity and may include changes in skin color, skin temperature, swelling, and sensitivity. CRPS-I, formerly called reflex sympathetic dystrophy syndrome, is diagnosed when confirmed peripheral nerve damage is not present. CRPS-II, formerly called causalgia, is diagnosed when there is confirmed peripheral nerve damage.
Delayed onset muscle soreness, or DOMS, is muscle pain, stiffness, and tenderness that appears about 12 to 24 hours after unaccustomed or intense physical activity. Symptoms usually peak between 24 and 72 hours and gradually resolve over several days. DOMS is a normal physiological response to increased exercise volume or intensity, especially eccentric contractions, where muscles lengthen under tension, such as downhill running or lowering weights.
Autoimmune, connective tissue, and neuromuscular conditions
Myasthenia gravis, or MG, is a chronic autoimmune neuromuscular disorder that causes skeletal muscle weakness. The immune system produces antibodies that attack acetylcholine receptors on muscle cells, reducing the ability of nerves to stimulate effective muscle contraction. The testable mechanism is impaired neuromuscular communication at acetylcholine receptors.
Ehlers-Danlos syndrome is a group of genetic connective tissue disorders involving defects in collagen. Different types can affect the integrity of skin, joints, blood vessels, and other organs. Massage therapists should work gently with these clients to avoid hyperextending joints, damaging fragile skin, or stressing blood and lymphatic vessels.
Immunosuppression is reduced activity of the immune system. It may occur naturally because of conditions such as HIV/AIDS, cancer, malnutrition, severe illness, or poor sleep. It may also be intentionally induced with medications or treatments, or occur as a side effect of medications needed for other reasons. Immunosuppressed clients may be more vulnerable to infections, so hygiene and illness screening are especially important.
Raynaud’s syndrome involves episodes of reduced blood flow to body parts, usually the fingers and toes, in response to cold temperature or emotional stress. These episodes can produce color changes, discomfort, and in severe cases tissue damage. The core association is cold or stress-triggered vasospasm in fingers or toes.
Endocrine and metabolic conditions
Type 1 diabetes mellitus is a chronic autoimmune condition in which the immune system destroys insulin-producing cells in the pancreas. This results in little to no insulin production. Without enough insulin, glucose remains elevated in the blood instead of being used effectively by cells for energy.
Type 2 diabetes mellitus is a chronic metabolic disorder characterized by insulin resistance, meaning the body’s cells do not respond effectively to insulin. Blood glucose becomes elevated, and the pancreas may not be able to produce enough insulin to overcome the resistance. Type 2 diabetes is the most common form and is often associated with lifestyle factors such as diet and exercise, though genetics also matter.
Gestational diabetes mellitus is diabetes that develops during pregnancy in someone who did not have diabetes before. It involves elevated blood glucose because the body cannot produce enough insulin to meet the increased demands of pregnancy. It often resolves after delivery, but it can increase risks for both the pregnant person and the baby.
Pregnancy, breast conditions, and reproductive conditions
Pre-eclampsia is a serious complication of pregnancy involving high blood pressure and signs of organ damage, often involving the liver or kidneys. It can progress to eclampsia, which includes seizures. This is an important medical red flag, not a routine pregnancy discomfort.
Mastitis is inflammation of breast tissue, often with infection. It commonly occurs during breastfeeding but can also occur in people who are not breastfeeding. Symptoms may include pain, swelling, redness, and warmth of the affected breast, and it can progress to abscess formation if not treated.
Mastectomy is surgical removal of one or both breasts, partially or completely, usually to treat or prevent cancer. The type and extent of surgery depend on diagnosis, cancer stage, and risk factors. For massage therapists, this term is relevant because mastectomy may affect scarring, shoulder mobility, sensation, lymphatic drainage, and client comfort with positioning and touch.
Endometriosis is a chronic condition in which cells similar to the uterine lining grow outside the uterus. It can cause pain and dysfunction in reproductive and digestive organs. Symptoms may fluctuate with the menstrual cycle, but the condition can affect daily life beyond menstruation.
Heat illness and temperature regulation
Heat cramps are painful involuntary muscle spasms that occur with intense physical activity in hot environments. They are often related to dehydration and/or electrolyte imbalance. Heat cramps are the least severe of the three levels of hyperthermia in this deck: heat cramps, heat exhaustion, and heat stroke.
Heat exhaustion is a form of hyperthermia characterized by excessive sweating, headache, vasodilation, and dehydration. Excessive sweating may lead to nausea, vomiting, low blood pressure, light-headedness, fainting, and skin that is cool to the touch. Heat exhaustion is more serious than heat cramps and can progress if the person is not cooled and rehydrated appropriately.
Heat stroke is a severe, life-threatening condition caused by prolonged exposure to high temperatures or exertion in hot environments. Prolonged dehydration may lead to lack of sweating and shock from water and electrolyte loss. The person may become confused or delirious. It typically occurs when core body temperature reaches 104°F, or 40°C, or higher. Heat stroke can damage vital organs including the brain, heart, and kidneys and requires immediate medical attention.
